Mobile Pantry for Clients

Agape Family Health is a Federally Qualified Health Center (FQHC) and a Federal Tort Claims Act (FTCA) Deemed Facility, covered by professional liability insurance through our enrollment in the Federal Tort Claims Acts program. For further information, please contact our AFH office at (904) 760-4904.

No individual or family will be turned away for inability to pay. We offer a Sliding Discount Program that reduces the cost of care to our uninsured patients based on their income and family size. Patients with insurance may also benefit from this program.
