On April 1, 2016, AGAPE Community Health Center, Inc. was independently awarded the designation as the only 330 (e) - FQHC in Jacksonville, FL.
AGAPE operates four (4) Comprehensive Primary Care Centers, one (1) Women’s Health Center (AGAPE – Dunn Avenue), two (2) 403 b pharmacies, and one (1) Community Partnership School (Ed White High School) which is located in an underserved community.
